What Is Community Nursing Care?

This NDIS-funded service is designed for participants who require nursing support due to ongoing medical conditions, recovery from illness, disability-related health needs, or assistance with treatments that cannot be provided by a support worker alone.

Our Nursing Services Include

Wound Care & Management - Professional care for surgical wounds, pressure sores, and chronic wounds.

Medication Management - Assistance with medication administration, injections, monitoring, and reminders.

Chronic Condition Support - Nursing support for conditions like diabetes, epilepsy, respiratory issues, and more.

Catheter & Continence Care - Safe management of catheters, stomas, and continence-related needs.

Subcutaneous Injections - Including insulin, vitamin B12, or other prescribed injectable treatments.

Post-Hospital Care - Transition support after surgery or hospital discharge to ensure safe recovery at home.

Health Assessments & Reports - Ongoing health monitoring, vital signs tracking, and reporting for plan reviews or medical teams.
